Important to note: Please be thorough in your answers. For #6, make sure that you answer ALL questions. In addition, evaluate the US against two other countries in terms of cost, equity and access (meaning: compare the US to one other country on all three dimensions, and compare the US to a second country on all three dimensions. A chart for this is acceptable).
