Fabricated Intake / Social History Report 

Fabricated Intake: This is a fun assignment! Approach it as such because you are going to get to practice your clinical skills!

****Please note that the material you need in order to complete this assignment will be covered over the next few weeks. You may not be able to fully complete this assignment right away as we will be learning a lot of this information during this section. It is due Friday, April 28, 2023 at 11:59pm.

Step 1: Find a friend or family member to interview. Ask them to make up a patient who is struggling with a mental health problem. 

Step 2: Use the “Intake Assessment Form” on pages 460-467 in the hardcopy book to guide your questions. This is for you to take notes on but you will NOT turn them in. You will take them and then write a fabricated intake report in paragraph form.  You can find this in the electronic book by opening the platform and clicking on the search bar in the upper right-hand corner. You will be able to search by page using this feature. 

Step 3: Using Word, you will write a full intake report. 

      A. Place a “Header” which states “FABRICATE REPORT BASED UPON FICTICTIOUS INFORMATION.” 

     B. Write a report in full paragraph form, detailing the information you learned from your interview. There is an example on pages 307-309           in your hard copy book. This is called a Social History or “Intake” Assessment. You should include information about childhood and           family history, medical history, social history, employment history, summary of symptoms/history of the mental health concern, drug           and illicit substance use, suicide risk, homicide risk, a mental status review, etc. End your report with a summary and recommendations for treatment. Your final           report should be between three and five pages. I am not worried about length but about capturing all necessary information. 

Step 4: Send a draft to [email protected] if you want me to review it by April 21, 2023. This is not required but something I offer to make sure you get the most out of this assignment. 

Step 5: Upload your final word document to this assignment which is worth up to 70 points
