A:

Although there are provisions for standardized care, public funded health care programs do not overrule cultural differences. There must still be respect for cultural differences and patients must still be provided care without discrimination. According to Tikkanen et al. (2020), there are several federal offices who are tasked with developing policies to reduce disparities. The Office of Minority Health creates policies related to racial and ethnic minority groups. The Health Resources and Services Administration provides grants for low-income and underserved populations. Lastly, Indian Health Services, which is funded by the federal government, serves American Indians and Alaska Natives.

 

Cultural differences as well as language barriers absolutely influence compliance within health care. The Lee family does not believe the medication regimen is helping Lia because they believe in shamanistic healing and sacrificing animals to save the health of the Hmong people. They believe the causes of illness to be spiritual causes such as loss of soul rather than natural or environmental causes. Their language barrier prevents them from understanding medical direction; therefore, Lia does not receive the correct dosages of medication. Often when visited by home health clinicians, Lia’s medications were expired, or the dosage scheduling was not being followed. Their beliefs allow them to believe they have the authority to choose when and how they provide her medications. For example, Lia’s father was double dosing then her mother discontinued her Tegretol because she felt as though it was too strong in combination with Phenobarbital. These medications were essential to controlling Lia’s seizures. This eventually led to her physician writing a letter to Child Protective Services who removed her from the home.

I do not think public funded healthcare program overrule cultural differences. There is now a need more than ever for a safe, patient centered care to improve health outcomes as professionals provides care to the increasingly multicultural society. Health care practitioners lack the cultural competence to understand and interact effectively with people from other cultures. It is vitally important to eliminate health disparities and social disadvantages for all patients, regardless of their ethnicity or race.

 

To avoid the medical mishap that impacted the Lee’s family, public health funded programs must ensure that clinicians are able to understand how each patient’s sociocultural background affects heath beliefs and behaviors. Healthcare practitioners are able to provide better care for multicultural populations by understanding the patient’s social determinants health. There should be a clear communication between patients and health care giver so that providers can collect accurate medical information, this will encourage active dialogue in which patients and health care practitioners can ask questions, correct misunderstandings as well as build trust. Miscommunications, lack of trust through language barrier are some of the factors that contributed to the Lee’s family misfortune. Therefore, public health programs are to be cognizant of these and put some measures in place such as acknowledge diversity, employ a culturally and linguistically diverse staff as well as orientation for new staff and ongoing training for employees.
